Youth Declaration
STALBRIDGE YOUTH FOOTBALL CLUB AGREEMENT Season 2009/10
Following are areas that you need to be aware of and you will be required to sign to state that you are happy with the detailed information.
Parents must inform the secretary of Stalbridge Youth Football of any health issues their child may have.
Parents are responsible for the drop off and pick up of their child within the training time schedule and that all fees are paid.
Parents may drop off/pick up their child under the full understanding that other persons may have to attend to their child if there is an accident.
Parents must ensure that their child is dressed accordingly i.e. shin pads etc and has an adequate drink with them.
In the event of any home or away friendly matches organised parents are responsible for the health & safety of their child including the transportation to away matches. Fees for this season are as follows:
A ONE OFF FEE OF £15.00 per child per season for training ai payable.
A) I as a parent fully understand that in the event of any type of accident concerning my child I cannot hold any other person responsible, whether it is another adult or child attending the sessions, as I am fully responsible for my child.
B) I as a parent fully understand that in any event concerning the football goals or other equipment I cannot hold any other parent or child responsible for any accidents to my child.
C) I as a parent can not hold any claim over the football club or parent/child for any accident concerning the football pitch. Example, cuts or bruises or breakage of bones due to stones or other items.
D) I as a parent take full responsibility over any first aid requirements, such as bangs or knocks to my child’s head.
E) I as a parent give permission to the coaches to check for any injuries to my child. Upon recognising an injury, I as the parent must take full control over the injury and any treatment required.
F) I as a parent understand that my child will not be allowed to play competitive football without signing on for Stalbridge Youth F.C and paying the signing on fee of £5.00.
